I know two people whose annual physical blood work showed problems that turned out to be serious. However, my husband says his dad died of a sudden massive heart attack one week after a clean physical, so who knows. out of curiosity do you know what indicators were and if they had any symptoms? She said if there had been changes from last year she would have done bloodwork or if I had any complaints. Both of these people had low iron as an indicator. Further tests revealed that one has a rare form of leukemia and the other has a tumor in his colon. Neither had symptoms.

EKG is not indicated for screening for younger healthy adults with NO risk factors. There are calculators that can determine risk and from that, an EKG may be indicated.
As far as the original doctor, they were just doing what they were trained to do if you have zero risk factors. An EKG was once normal screening but the evidence has not born out a sufficient benefit.
That said, it's the same organization that says there is no benefit in screening mammograms under age 50 on a cost-benefit ratio. In the end, you do what you feel is right by your patients. My own internist does an EKG on me every year (I am not without risk factors, especially with a mother who had a heart attack at an age younger than I am now... wow that really hits me) She does a screening urinalysis each year too and honestly, I do not see the point in that in an asymptomatic relatively healthy patient.
