ICU psychosis is a very real condition. I am sure if he had a CABG, he had a foley catheter at least for several days. That can be a source of an infection. You mentioned he didn't have a UTI but it might not hurt to ask when that urine was tested.
I would certainly ask the nursing staff if the agitation has been discussed w the MDs. If not, you need to make sure you get an opportunity to speak to his primary provider. The unit may have its own nurse practitioner that would be an excellent resource for you, too.
The psychosis or delusion is typically caused by not having a set schedule and basically d/t lack of sleep. Pt don't get a sense of day or night so it is very difficult to get the REM sleep. That's it in a nutshell. Be sure to talk to the staff about other sedative options so he can get some rest at the appropriate times.

Another RN chiming in. When I was a brand new nurse I took care of the sweetest little lady...until she came out of the OR after a colon resection. It was like a completely different person. At times, I expected her head to spin all the way around and fire to come out of her eyes. Pretty scary stuff for a new nurse to handle. It went on for about a week and I went into her room one morning to get her up to start the day and she was back. She was absolutely mortified that she had acted the way she did. Her family had told her and she didn't believe them so she asked me. Poor thing had no recollection at all.

It happened to my elderly mother too one night she had to spend in the hospital. She was totally hallucinating about people being in her house. She did not sleep all night. When she was released she was normal again. I don’t think it has anything to do with UTI but rather age related issues, perhaps it brings forth a touch of dementia.

Again, thank you for all of your helpful responses. We brought my dad home today after reading here and on other sites to bring my dad home from the hospital. His medical team and the hospital social worker agreed also that being in a familiar environment and following his normal routine would be our best choice at this time. We have set up a schedule for 24 hour care so my mom will never have to care for him alone. He had a horrible night in the hospital last night and I could not keep him in a place where he was so scared and agitated.
The two things he kept asking for were to be home and to take a bath. He was able to do both today and we sat at the table for dinner and he was able to join in saying grace. The routine of this caused him to visibly relax. Hopefully he will be able to get some peaceful sleep in his own bed tonight. If not, we have family members lined up to walk with him all night if that is what soothes him. We just want him to feel safe and secure.
He has provided us with a few chuckles during this difficult time. Today before we left the hospital a Eucharistic Minister came to his room offering communion. “I just finished eating.” My dad replied. That’s fine the EM said. “Why don’t you join in at the table of the Lord?” “I don’t know.” says my dad. “What’s He having?”

That’s fine the EM said. “Why don’t you join in at the table of the Lord?” “I don’t know.” says my dad. “What’s He having?” I literally LOLed. In April when my 82 year old mom fell and broke her hip had surgery to fix it then diagnosed with colon cancer and had surgery for it, she hallucinated badly. Rats in the walls and bats flying in the room. So so real to her. They upped her fluid intakes, she was given more blood transfusions and some drugs were changed. She remembered none of it a few days later.
