I've had some bad scratches and was fine. I DID get bit badly once on the wrist. I soaked it Epsom Salts and cleaned it with hydrogen peroxide. However, the swelling didn't really go down so I did go to the doctor who said I had a mild case of CSF, but that Id done the right thing with cleaning it and all it took was some antibiotics.

I've had my share of scratches and bites too - the latest one is 2 weeks old when my cat decided to twitch her tail right where I was stepping and she bit me through my jeans. One small puncture and two inch-or-so-long gashes (well, gash makes it sound really bad, but it was enough layers to bleed). That one I went to the restroom and washed then used peroxide on, then made sure it was going to stop bleeding then applied antibiotic ointment on it. It was red and sore for a couple of days and is still a little tender (more bruise-ish feeling) now, but then it was just above my ankle where there's not much meat. The only time I've ever had to have medical intervention was when our other cat bit me (as in sank her fangs into my thumb then freaked out that she had done it and just sat there with my thumb stuck in her mouth). She wasn't feeling well and I was trying to feed her and she missed the piece of chicken. After I got her to the overnight emergency vet clinic I went to urgent care and got it cleaned out and strong antibiotics because it was already swelling more than a regular scratch or bite within a couple of hours of the bite happening, even with squeezing it and peroxide and antibiotic ointment prior to taking the cat to the vet.
I'm sure the shoes are aggravating it more than it being a matter of a cat scratch. Hope it stops bugging you soon!
