Mom of four here.
First things that came to mind:
Buy blue/masculine colored receiving blankets, clothes, socks, etc.
Tee shirts and onesies in size 9months & up that say stuff like "Little Brother."
They sell month # stickers that mom can put on a onesie to take a photo of her baby each month to document age. I've even seen these at Walgreens, so they are everywhere.
Give a gift certificate for photos to be taken. JC Penney's offers a portrait club membership that is something like $30 for a year or two of free sittings.
Have a blanket embroidered with his name (after he is born and named). Babies born later in the line-up often have fewer personalized items.
Buy him a dress outfit. Mini suit for 12 months. Something nice he can be photographed in.

Dr.Seuss Oh, the Places you'll Go! They could have their teachers sign it each year, K-12.
Another gift I like to give are the small Dr.Seuss board books. Nose book, Teeth book, Eye book.

Since this is a boy after two girls, I would take all kinds of boy outfits, blankets, bibs, etc. Also, I would take larger size clothes since babies grow so fast.
My young friend had a baby boy recently, and he couldn't wear most of his shower gift clothes because he weighed almost 10 lbs. at birth(much larger than expected).

Ask if she has a rock n play. If not I would say buy them a Rock N Play. This was the most suggested idea when we were having twins. Having been there and done that I HIGHLY recommend this item! I'll second this. My DD *lived* in ours when she was really little, and she would even sleep in it occasionally when she was having reflux issues. Worth its weight in gold! We had reflex as well, and it was so helpful. The feature that I LOVED was how it collapsed so we could put it away, or use it when we went to visit family and friends!
