Which way does the door open? Typically I will open the door whichever way it opens, stand aside and hold it open, and let the other person go through. But if the door opens in I’m inside, and if it opens out I’m outside.

Totally depends which way the door opens. If it opens inwards I hold it open for them. If it opens outwards, then I go out and hold it open.
Like, there is literally no other way to do this. Unless you try some weird gymnastics where you lean all the way over trying to hold open a door that pushes out.

I know at B&N I push the door to enter and exit. As to the OP’s question, my response is like many others, it depends on which way the door opens, and how close/far away the other person is.

Depends on how the door opens. If it pulls in, I’ll hold it open for them and then exit. If it pushes out, I’ll exit first and then hold it for them.

If the door opens out, I go out first and and then hold the door. My arm is not long enough for me to hold the door and let them in without invading each other's space, unless it is a small child.

For me, it depends upon how close they are. If there is time to go through the door and then hold it open, then that is what I do. If they are closer, then I hold it open on the inside to let them through.
Of course, if my husband is with me, he does it for them. I constantly loose him at the door as he holds it open for people who just keep coming in, one group or family after another. So I have to wait for him to catch up when everybody in sight is inside. LOL

Same as everyone else in relation to whether the door opens inwards or outwards, except that now with a 2m social distancing and only a certain number of people allowed inside the shop at one time, it's more difficult.
In the pharmacy today there's a maximum of 2 people allowed in at a time, so if you're person #2 exiting, you have to exit before person #3 enters. You can't then hold the door open because then you're closer to them than 2m. So you smile, thank them for waiting and apologise for not holding the door. New Covid etiquette!

Most stores I frequent have either automatic or revolving doors, so no one is holding them for anyone.
Considering COVID, I’m not usually holding doors (or wanting others to hold a door for me) because I’d rather have distance from people I don’t know. Unless the person was standing right there, I’d try to move quickly out of the way.
If I exited a door that swings out and someone was standing at the entrance, I would hold it open for them after I exited.
