I had the same problem with my son. We had the best luck at Pacsun and American Eagle.

freecharlie, I would definitely try Old Navy online. My 19 yo son has a 29/30 waist with a 36 inch inseam! He is very tall with very long legs. Can't go the shorts route with him because he goes to school in Flagstaff and it snows. Lands End and Eddie Bauer also had some unusual sizes, but they were pricier. Good luck!

We had that issue last year, thankfully ds bulked up a little and is up to a 30" waist....much easier I got lucky and found a couple pairs at Plato's Closet, I think they were American Eagle and maybe Aeropostale. Penney's carries them online, but I never found them in store. Honestly ds mostly wore jogging pants anyhow, so thankfully I didn't have to make a part-time job out of finding them for him Plato's Closet has male clothes?

Most of them are more trendy than my Nike-wearing kid likes but jeans are jeans.

My boys both have long legs and thin waists... I noticed when I had to buy dress pants for graduation that Kohl's had more sizes than the store. I think you'll find that with most stores, if you shop online you'll find more sizes.
