Genital gangrene is real.
Omg, i am just so stunned that this could actually happen to a person, Taking a drug that they thought would save their life.
I can only hope that there were only a couple of cases.

Gangrene is the result of no/very poor supply of circulation to tissues. When no blood & therefore no oxygen is supplied to tissues they die. So it is possible to get gangrene in any area of the body if circulation to that area is severely compromised. A drug to help treat diabetes isn't really a "life saving" drug. Plus these people already have diabetes which predisposes them to poor circulation and infections, and this particular diabetes drug seems to also increase the risk of Fournier's gangrene.
So it isn't as simple as "we gave you this drug to save your life and now you have gangrene of the genitals" People with diabetes already need to monitor themselves for infection & evidence of poor circulation...especially their feet, so they will just have to be extra vigilant with their genital areas as well if they are on this drug.

It's called Fournier's gangrene, and it is awful. No direct experience with it, but I review medical records in my job and from what I have read and researched it is a terrible thing.

Is it more common than I would think it is? Probably not in the general population. It's still rare - I've never seen it. However if you are just looking at older people (particularly men) with diabetes who also have other health issues and less than stellar hygiene practices - then it might be. It's interesting that class of diabetes drug seems to increase the risk of this. It's likely that now doctors will try to avoid giving it to those patients who are already most at risk.

it looks like an entire class of drugs could increase the risk; I found this article: FDA- Cases of Rare Gangrene of genitals tied to Diabetes drug classbut it's not really that recent of an article, and it says the FDA recommends a warning on the medication prescribing information, and that doctors should assess for that condition if patients present with certain symptoms that could be tied to the gangrene. (and I am STILL not going to google images!!)

I have had two patients in my career with Fournier's gangrene and let me tell you that is the only thing I've ever smelled that made me vomit. I'm not talking gagging either. I can't even give you something to compare it to. I swear that ER smelled like that for three days after. It was awful. Gah. Makes me gag just thinking about it now.
