I think I'm going with the family gift basket idea for my bro and SIL. Long history of gift giving hell with these people so the family gift is the way to go for me.
Have you seen any gift basket ideas that you thought were clever and helpful?

I'd try to center the basket around something they enjoy as a family. If they like going to the movies, for example, I'd include gift certificates to the movies, popcorn, candy, snacks, etc. If they like camping, maybe a gift certificate to a place they shop for supplies (REI, Bass Pro, Wal Mart), and some camping snacks/small useful things. If they like nothing and are hard to buy for....alcohol, snacks and chocolates. I guess I think snacks are very important. But, really. What kind of jerk would complain about free snacks? (I actually know a couple of jerks who would!)

Are there kids in the family? Ages?
Game night Outdoor games Grilling Alcohol Fancy oils/vinegars/spices for cooking Tailgating equipment Movie night Video games Book lover
One idea is to look at the different gift baskets that are out there. If you don’t want to buy it oremade, you could just copy it and assemble it yourself.

They are indeed rather jerkwad-ish about gifts. They also live in the middle of nowhere and SIL has a ton of rules that change frequently. Long history...

I put together a fun gift basket filled with hot cocoa goodies. I found a 2 pack of mugs in the Target $1 spot, then added a bunch of single cocoa mixes, a cute pack of candy cane marshmallows, some of the chocolate dipped pretzels and then I added an electric frother for mixing.

Cocoa mixes ( most Swiss miss) don’t work for us. Makes our faces break out in huge zits , but oddly the Mexican hot chovolate mix for winco in the bulk foods doesn’t.
Die hard movie with tons of movie candy and popcorn. Best Christmas movie ever.

I'd be tempted to be all passive aggressive and break all the rules this year. It may end your gift exchange (which sounds like it would be a good thing.)

What is your budget?
A gift certificate for a family dinner out? Pizza, Chili's, Dave & Busters, whatever their favorite is.
Family outing for Mini-Putt.
Tickets to G-League basketball game. Or minor league baseball game. If, of course, they have a team in their city.
Day lift tickets to local ski resort.
If you had your heart set on gift basket I'd do a bunch of snacks and drinks that they can take along to whatever sports they do.
ETA: Tickets to G-League/minor league games are inexpensive and you get to see up and coming players. We go to a lot of G League games because we have a local team.

We are just grossly different people with grossly different values. It's a very long history and she has made it as painful as possible for years. I could write a novel with their response to me forming a relationship with their children. They have zero relationship with my children and do not try. I could be like that but I can't. I am a dutiful aunt and ignore their parents so that when/if the kids ever become aware of the world they will know they have a safe place to land. In that case, how about a basket full of coal? hahahahahaha! I'd paint it pink because pink is against the rules.

Honestly if someone was that much of a PITA to buy for I would give them a goat. Well I wouldn’t technically give THEM the goat, but I’d find one of those charities that give goats to impoverished families in third world countries and donate one in their family’s name! I’m sure that would break ALL the rules! But I’m kind of a rebel like that.
