You could make that in a program like Inkscape in about five minutes, tops.
Heck if you want, I can whip something off real quick.

Took me longer to nab a reference picture, set up Inkscape, and get this into Dropbox than it did to make. |

Heck yeah everyone's free to download it, didn't even think it bore mentioning. I'm happy if anyone finds it useful, given it's just a few rectangles in a box. Easy-peasy-lemon-squeezy.
SVG is great. Its short for "scalable vector graphics", which means an image that is defined with math rather than pixels. It can be changed to any size without losing quality, and with the right software you can alter individual parts of a bigger design too.
