StephDRebel do you have a teaching hospital near you? This needs looked at, in person, immediately. Can you print out the photos you have shared us, and date them? You need to be able to show them that it is moving and getting worse and have a timeline.

He got through to dermatology today, theyre supposed to call back tomorrow woth an appointment this week. I'll look for a tropical disease specialist! Insist they look at you pictures!! If you cannot get into Tropical disease, please try infectious disease.. It IS getting bigger as in moving!! Please protect yourself too! This IS urgent!! He's been to infectious disease twice in the last 5 years with no answers, but they were before this presented. We're trying to circle back around there. Derm looks like it will be by friday. I'm not touching it and scrubbing down after putting stuff on it. He keeps it covered and we've been using a topical antifungal. We are at almost 2 months since it started and so far it does not seem to be spreading further than that spot on him but it's certainly bigger.

He has been seen in person by the department of neurology and rheumatology at Ohio State and that is who his dermatology appointment is with. He sent them all in, dated today and they said "Oh, that is really odd. I've never seen anything like that." and they're calling him tomorrow with the earliest appointment.

That second photo is spreading from the first one. I would insist on being seen or present to the ER with the photos.
Use a marker and draw around the entire edge and keep a close eye on how it is moving.

Ok, so two months ago was about June. July. And if it is a parasite, they can give inside you for about a month before 'seeing' them. So did you do any kind of travel around early June? While hookworms are not common to get in the US, they are found in sand and at beaches (or if he's been laying around in the dirt...but I am assuming that would be a no).

He could have picked up hookworms from the dogs, which from the reading (linked to the pictures in Why’s post) seem to enter human hosts (which is rare) through cuts in the skin.

I had a rash after surgery from the tape and antiseptic used to clean the area. Looks similar. I haven't read all of of the posts, but did you use a steroidal cream to help with inflammation?

His surgery was the day after Memorial Day. Nothing but doctors appointments, we didn't leave the house other than grocery pickup from March to July. The last time he was at the beach I think would have been Puerto Rico a year ago in August and before that Hawaii the March before. It started appearing pretty soon after surgery, we thought it was irritation from the tape initially. I feel like we noticed it less than a month after, it was late enough that it thought twice about it being tape but it was still possibility enough that I wasn't super worried. No dirt laying that I'm aware of. He does work on complex wheelchairs so there is some exposure risk to things but it's just so odd that it's right on the incision. It feels like it almost has to be associated.

They did steroids, antifungal, anti viral, and antibiotics in large doses and did a topical steroiod, antifungal cream, tea tree oil, and vitamin E. And, it may be itchy, we aren't sure. He feels deep pain in the muscle/bone/tissue/somewhere but doesn't have feeling in the area of the incision.

Are you touching it every single day? You might cover it to protect it and just let it be and see if it improves over a week.

I say Scabies. Both my kids had it once. They had lines like that from the mites tunneling under their skin. Good luck figuring it out.

Mine looked NOTHING like this and it itched HORRIBLY. I had it on the backs of my legs and I scratched it so badly that my legs looked like hamburger! I also had pityriasis, which is a virus, and it looks nothing like that the rash on Steph’s DH. I’m going with parasite.

Looks an awful lot like patient cases we studied in microbiology/parasite rotation.
My thought was hook or ringworm type of parasite.

I would also ask if there's a possibility that it is related to his MS.
The rash may be the physical manifestation that you can see, but you also said he feels a deep pain in the incision area? It makes me think that his own immune system may be attacking his nerve endings that were affected due to injury and/or surgery.

Definitely draw around the border of the rash. The rate at which it enlarges can help make the diagnosis.

My guesses were shingles and ringworm, but neither look quite right (the rash is on both sides and shingles is usually only on one side, and ringworm looks different than that). 😕
Sorry I’m not more help
I hope they figure it out ASAP and your sweet husband gets some serious relief soon.

It makes me think that his own immune system may be attacking his nerve endings that were affected due to injury and/or surgery. They have never been able to confirm the MS diagnosis. They were doing the MRI to look for the lesions in his spine since they aren't in his brain and found the injury instead. The surgery alleviated a lot of the symptoms that they were attributing to the MS potential but things are changing again as the nerves do whatever they're doing. his taste and smell are close to normal again but his pain levels are off the chart and he's had two instances where his hand has swollen huge and then gone back down with steroids. the pain doesn't seem to be the whole incision area, just about where this mess is. He doesnt have much feeling at all topically around the incision site but he has had awful back pain since a few days after surgery.

It doesn’t look like a tape burn. Did they even use tape on the incision? Or did they use an aquacell? I’ve had a rare amount of pts be allergic to that.

You should email Dr. Lisa Sanders who does crowd sourcing with the New York Times. There's a show on Netflix about it. Lisa.Sandersmd@gmail.com
