We've never ever once voted by mail and have never ever once requested the ability to do so. We have always voted in person. Imagine our surprise when a mail ballot showed up this week. He got one, but I did not. They have to be requested. The only thing I can think of is someone requested it, hoping to intercept it maybe? Oddly enough, we're both registered as no-party affiliated (Arizona speak for "Independent.") I get mail from the Republicans and he gets mail from the Democrats. Never the other way around. So it makes me think that whomever processed this requests is accessing one of those lists that has separated us into those two groups. (Note: I am not accusing the Democratic party of any wrong doing-just suggesting that someone may have bought the same lists they have). PSA: I do know that mine has not just been stolen because we are enrolled in Informed Delivery with the Post Office which sends us an email every day with images of the mail we are receiving that day. If you're not enrolled, I suggest you become so. It's free on their website and then you can watch to make sure your ballot arrives as it should.

Are you absolutely sure it's an official ballot?
I kept seeing complaints of people getting ballots that weren't requested or for people who didn't live there. When pressed, they posted a picture of an application. Some of them were actually sample ballots from the parties and were likely going off old mailing lists, but not the actual ballot.
DS keeps getting letters about applying for an absentee ballot. He did but it was rejected because his signature didn't match. Now those form letters (not from the state, but from the Democratic party) get tossed into recycling.

Can you contact your Board of Elections office and ask about it? When I requested my ballot, I could track my request so I'm sure they have record of someone requesting it if indeed it happened that way.

Yes-I assumed it was a sample or something at first too and almost tossed it because we usually download them. Then I remembered our samples already arrived and so I opened it up and sure enough, it's the official ballot.

It says "mailed 10/8" I checked mine and it correctly says "You're not signed up to vote by mail."

We got our mail ballots this weekend. The ballot comes from the County Recorder. The outer envelope (for AZ) is green. Actually, Pima county is green. You are in Maricopa right? I don’t know what color that would be. I guess I am not really any help. But just wanted to mention that I received my ballot. I am on the permanent early voter list. Don’t remember signing up, but have been receiving mailed in ballots for years now.

A ballot is not really a vote until it is mailed in, verified as coming from a registered voter, etc. You can have a house full of ballots that will not affect anything until one of them is mailed in and goes through the process. Once it is accepted, your name is marked as having already voted and then you cannot vote on election day.
A neighbor works the tables on election day and she said when you go in to vote your name on your voter registration card is compared to the list of voters and those who have already voted by mail are marked and are not allowed to vote. If you feel this is in error and wish to contest it, then you are allowed to fill in a provisional ballot which then goes to the judge who determines if the vote is accepted or not.
All this to say, not to worry, Whoever sent the ballot to your husband was probably not trying to steal his vote. Check to see if there was anything filled out for him on the ballot, any personal information which would have made it possible for somebody to claim to be him.

If you request a mail in ballot but decide you want to vote in person, you must bring the ballot with you to the polls and surrender it to the judge of election. So in the situation MissJen described, my suspicion would be that someone hoped he would throw it away and then have to vote provisionally on election day.
