I'm willing to buy one (but not an ongoing cost) but would like to find one that has a good database and automatically categorizes items when possible.

The Alexa app from amazon. I already use the echo to make my list and it’s also easy to add stuff from my phone. It does separate things by department, sometimes I have to specify if something is frozen or canned to get it to the right category but for the most part it does just fine. And the app is free.

I just use paprika. It organizes things for me, is easy to add custom items and has a great database.

I’ve been using the Our Groceries app for years. Recommended by peas. I’m pretty sure it was free, but I might have given them $2-$3 to get rid of ads.
I love it ... very customizable. I have about a dozen ongoing lists for different stores, including the hardware store, the craft store, Penzey’s, etc. Also it syncs with any other device I tell it to, so I can add things to the list from my iPad but shop with my phone, or you could sync it with your DH’s phone if he’s doing the shopping.
ETA I created the categories in my lists and I assigned items to the categories. I don’t know if there’s an option for the app to set up the categories ... I’ve been doing it this way for so long, I just don’t remember.

We've been using AnyList for a couple of years. We can share lists between us, which is useful as we both grocery shop at various times. It also allows you to add recipes, and you can add all ingredients from the recipes direct to your shopping list.

I kept getting a pop up about paying and I think it was a monthly fee? I've tried so many lately that I can't remember.

I use Paprika. There is an initial cost per device so I took advantage of their usual Black Friday deal. Categories already exist so items go into the app's best guess on where they belong. I buy enough weird stuff that I have to move things from the Misc category sometimes. I haven't played around with the aisles feature. I can create different lists for different stores and add notes like prices.

Back when my up close vision was better, this would have been nice to have! Now I just take a picture of my list because I always leave it at home!  But is an app something that can be used on a computer? That would be great for me to do a list on my computer, then print it out.

I use Our Groceries. I think it will work with Alexa too. I started using it that way, but I have found it is just as easy to do it manually.
I have different lists for different stores. Also, have categories set up for similar items. There are only 2 of so it’s all I need. I also set special lists for special occasions, such as Thanksgiving or Special bbqs

I use Paprika and I think it was from a Pea suggestion. I love that I can look up my recipes online, add them to the list and then on the weeks I want to make them you just choose the recipe and it brings up all the ingredients for your grocery list. You can also easily change the serving size for the recipes.

I use Alexa and my echo. In the kitchen if I find I'm low on something I say Alexa add salt to my shopping list. Sitting on the couch I remember I'm low on something I just ask Alexa to add it to my list. I have an Echo downstairs and one upstairs. In the bathroom I find I need toilet paper Alexa adds it to my list. When I open my app in the store there's my shopping list. I don't categorize. You can specify which list to add it to. Target, Walmart etc. I hardly ever type anything to my list. Alexa does it all for me.

My local grocery chain has its on app for online orders so I just use it and add to it through the week till I’m ready to submit my order. I do a combination of this and adding things to Alexa's list. (The rest of my family uses Alexa list and I'll use it if I'm in the middle of something and don't want to take the time to go into the app. On Saturday morning I look at what's on the Alexa list, add it to my grocery chain order and submit my pickup order. My grocery chain also allows me to add things to a list for in store shopping. It's nice because its automatically in the order of the store. When they redo their aisles, it's automatically redone.

On my Alexa app, there are three dots next to the word “Share” at the top of the screen. When I tap on the three dots a drop down menu appears with the option to show categories, hide items, clear, etc. Maybe this might help make Alexa more useful for you.  If I want to add an item from a specific store, I just say it when I add it: Alexa, add Costco waffles. Alexa, add Trader Joe’s wheat bread.
