Basic (stupid?) question on Passport Renewal Form DS-82

Thanks for the various responses. I decided I was overthinking question 10 and answered it with last name, first name and middle name since that is how it shows on original passport. You have two other opportunities on the renewal form to provide your name so I'm sure they'll figure it out.

Although it does appear that passport/renewal passport processing is getting much faster I went ahead and paid $60.00 expediting fee and $18.32 for 1 to 2 day delivery back to us in addition to the $130.00 passport renewal fee (these are all per person). I sent the applications together in one envelope priority mail for $8.95 which includes tracking confirmation. You do need to return your original passport and include a check for payment. I did two separate checks because I couldn't easily get both our names and dates of birth on one check.

Our original passport photos were done at a post office and we loved both our photos. We had the new photos done at Walgreen's and our photos look like convict mugshots. The poor lady helping us had to call the manager because the machine kept saying our photos were tilted ... major reason for photo rejection. Manager said he always has applicant sit in a chair while he takes photo and the machine didn't reject his photos of us. DH is a very smiley person so it took three photos to get one of him not smiling.

Thank god we are in our 70's, these may be our last passports.
