sueg - it surprises me how doctors don't read the notes. My doctor (she is new to me, I have seen her twice so I will let it slide), she asked me at my last annual visit when my last period was. I said when I was 36. I had a hysterectomy. I am 52 now. In my case, it was somewhat understandable. The university clinics are walk-in for patients with public insurance, so they didn't know I was coming in until I got here. Then, the doctors basically take the next case on the list, so they have very little time before actually seeing the patient. I also think in clinics like this, they want to se the referral from the P or other originating doctor, and ask the patient what they believe the problem is. My regular GP and Gyn know what's in my notes.
