I was taking Metformin but once I got to the full dose, the side effects were awful. I asked the Dr. to switch me to the extended release. I took a break from it because I had a hysterectomy and some other repairs 3 weeks ago and didn't want to have any side effects of the medicine while recuperating. I'm getting ready to start taking the extended release Metformin this week and noticed the dr. said to take 1000 mg in the morning and 1000mg at dinner. But, everything I've read said to take the whole daily dose once a day in the evening. Just wondering if you take this, how you do it? Also, with the regular Metformin, she had me ramp up the dose by doing 500mg with dinner for a few weeks, then adding in 500mg at breakfast for a week, increasing to 1000 mg at dinner for a week and finally 1000mg am and 1000dinner. Did you ramp up with the extended release too or just start taking the prescribed dose? Asking here and not the dr (yet) because I've asked her so many metformin questions via the online health portal over the 6 weeks I was on the regular metformin and then trying to switch and each time she'd reply it was shorter and shorter... I think I'm annoying her. Just wanted to get some info or ideas here before I go back and ask her how to do it. Thanks!

My husband takes 500mg extended release 2x a day per his doctor’s instructions. He never did a ramp up and has had no issues.

I take 500 mg twice daily - one in the morning and one at supper. When I was diagnosed as diabetic, I took the regular Metformin and had terrible gastrointestinal issues. The dr switched me to the extended release pills and I’ve had no issues.

I take 500 mg at bedtime (although it should probably be with food) and had no issues. My A1C was good after the first 6 months.
It is so much easier on my stomach than regular Metformin.

Taking the full dose once a day left me with some major low sugar levels in the hours following which is as bad as high sugar levels. The split dose kept levels on a more even number over 24 hours. I did not need to ramp up with the extended but I was compliant with taking the "regular" one so it was simply a matter of yesterday I took the regular and today I took the extended. I am not sure how it will impact you since you have been on a medication break.

I take 1000 of Metformin at bedtime along with the myriad other meds I take.
ETA: But I never had much of a problem with it anyway.

I’m on 2000mg per day, but I take it all in the evening. I used to have a split dose, but the dr. switched me over to once per day since I was forgetting to take my morning dose. I’ve been on it for 15 years or so with no side effects. The side effects really only happened when I was eating too many carbs, but YMMV. If you’ve been on a break, I’d start with 500, and increase to the 1000 over the week. Side effects will vary depending on the carbs you eat, and your level of insulin resistance. Everyone is different.
