I have used my main email address carefully for years now but somewhere, somehow it's gotten "out there" and the SPAM is out of controls. Is there anything that works to really stop the onslaught of SPAM that flows in by the hour? Or should I just resign myself to the fact that I need to cull out the real mail, update with a new email address and close the account?

I think you have to be diligent and forceful about getting rid of spam. I've set up "rules" in my preferences box for mail and I separately report all phishing emails to the underlying site. In the beginning, it took a bit of time. My email preferences let me set up any number of "rules", permitted a number of entries in each rule, and automatically imported the email address of the item I was writing the rule about. I found that a few entities were responsible for many of the spam email and once I'd caught their identifying information, their spam was never brought to my attention.
I think the biggest point is diligence ... until you get it under control!

I have an old hotmail account that I use for giving out on websites, two gmail that I use for more important correspondence, and my hospital email for work. I actually get the most spam at my hospital one- I never give it out so I am always surprised. I just don't set any filters in that, in the off chance that a patient email might get lost. I get a lot in the hotmail account, but it is almost immediately filtered into junk. I hardly get any at the gmail and what I do get is picked up before it makes it into my inbox.
Maybe just look at your settings and increase your filters? I spent a good month a long while back unsubscribing from things I no longer wanted and blocking and reporting junk and my inbox is much better now.

I recently threw everything that was in my inbox into a new folder and as new emails roll in, I either delete or unsubscribe. I ebay but have taken some time off from it recently, so I did set up a filter on those to go directly into a folder so I don't even see them right now.

I use and adore SaneBox. It is a subscription (but has a free trial), but it's so worth it to me. I had it for a couple years, then when trying to reduce the amount of online subscriptions/services I was paying for, I let SaneBox expire. I lasted 3 days without it and then renewed. I'd forgotten how truly hideous email is without it. SpamSieve is a good standalone product, but SaneBox is outstanding. It's worth it for the SaneBlackHole alone, for me.

I wish I could figure out who sold my addy because I used a gmail account for years to collect the crap. It's only been in the last couple of months that this became a problem. Ugh. Sounds like I have some work to do.
Busypea, I'll look into that as well.

I get a ton of spam too but Yahoo does a great job of filtering it out. Rarely does one get by. Then daily I just dump my spam box without ever looking at it.

I have a gmail address I use for friends/family that I won't give to my Dad (he only has my Yahoo address) because I know the minute I do, I'll get a ton of CRAP. Dad will click on anything and gets more viruses than anyone I know.....and that's only the ones we know about!

I get a lot of spam mail but gmail filters most of it out automatically (I never set up any rules). I hardly ever see any real spam in my inbox.

I just found that for some reason, I was asking Outlook to filter out the obvious spam.
Now, my next problem is the stupid IMAP setting. I can't find a way to delete emails from the server once they're downloaded to Outlook. It means going to the online web mail and getting rid of them there. And that's a royal PITA.

For any "legit" emails which I had opted-in (sale ads, etc), every few months I will go thru those and unsubscribe to the ones I don't really read or are no longer interested in. For actual spam, I mark the message as spam and add it to my blocked senders list. I don't get many actual spam messages, most are retailers who like to send messages nearly every single day.
