Not normally but I did do a couple as DT assignments for ruby-rock-it to promote their fancy-fold card blanks.

Not normally. I do either standard A2 or slightly smaller on a regular base. Just lazy I guess  |

The most intricate I do is a zig-zag fold with partial die-cutting or a gatefold card.
I have seen some really awesome looking box and pop-up folded cards though.

Love the Elizabeth Crafts pop up dies! So many great creative folds out there, but I rarely do them, I usually stick to the standard A2.
