Already bought a large, practical gift. Need something personal to go with it but am at a loss. She already got EVERYTHING she needs plus more. So looking for something small and special. Any ideas?

If you do crafting of any kind, a double photo frame which you paint or decorate in some other way would be a cool gift.
For my grandkids--and grand nieces--one year, I took plain wood frames and painted them, then decorated the first letter of the child's name and attached it to the frame. That was about 8 years ago, and some of them still have the frame on their dressers. They are 5x7 frames, so they can put their school pic in each year, but baby portraits the annual bd photo would work well for the little ones.

A book or two that suits them or is special to you, with a note inside. Useful, doesn't take up space and sentimental too!

As a twin mom, no joke, the nicest gift was someone to watch the babies for a few hours in the early days so that I could nap. So. Tired.

I liked the year of birth Christmas tree decorations for each baby -- that year it was a swan -- so pretty as a pair together on our tree.
I didn't really use the photo frame types of gifts -- they made me feel bad for not coming up with the perfect photos to put in them but life was just too hectic for things like that.
I LOVED the 6 months diaper service -- but I guess that doesn't count as a sentimental gift. . .

What about a gift for the Mom - something for pampering herself, even if she doesn't use it right away?

My 19 yo ds got a really cute gift that we still have tucked away Little thin fleece baby blanket which was embroidered with his full name, birth date and maybe his weight. Everyone loved it and I was asked more than once where it was purchased. I'm sure these days it would easy to find something like that online.

How about one of those silver necklaces marked with a finger print - you could use the combination of their fingerprints to make a heart? Two necklaces required, of course.
Etsy have different styles and examples.

As a twin mom, I might second the Christmas ornaments; I have one for my ODS, and one for the twins - yes only one.

I second (third?) the idea of Christmas ornaments. There are lots of creative ornaments for multiples on etsy, etc.

You could do a prayer for their wall. I recently purchased a printable from Etsy (which I could have done myself if I wasn't silly busy this month) and framed it - it came out very nice.

does the parents like playing sports or have a special activity they like to do?
we got a friends baby a little baby pair of golf shoes when she was born because the dad likes to play and watch golf. They loved them. I am not sure if she ever wore them but they have hung on her wall in her room since they received them.

Twin mom here - mine are 13 now. I like the Christmas ornament idea. Make sure to get two! I was in such a fog their first year that I only got one ornament for the two or them! I still need to find another one on eBay!!!
The other thing I liked and still have (in their baby shadow boxes handing in our foyer) are their embroidered baby blankets (like someone else mentioned). Or if you don't know their full names, maybe even just the shadow boxes so the mom will have them to put everything in. I love ours and I didn't get around to making them until a couple of years ago!

I think if you have time it would be fun and sentimental to get a book made that uses the children's names. You can get one for each child. Putmeinthestory.com is one of the companies that does this.
My son got a book like this and he loved hearing about himself in the story. In fact with his book I taped a photo of him in the back and his face showed on his page of one of the books.

I just had one kid, but one of the neatest gifts we got was one of those handprint ornament kits. You mix up the stuff, put it in the mold and press the kid's hand in it. There was a straw to make a hole to hang it once the clay air dries, and a ribbon to hang it on the tree. Because I have a bazillion stamps, I also imprinted the year into the clay before it dried. It's fun to look back and see how little her hand was!

I had twins. Items with twins are not impossible to find but you do seem to have to look harder. One thing that I really cherish is a Christmas ornament that has a boy & girl twin on it for their first Christmas. Yes only one one day I am sure they will fight over it. It's the only Christmas ornament I have seen for twins. I also had a Willlow Tree angel given to me that included two small babies. I will say we had the blankets - homemade quilts and ones with their names. We had everything in pink and blue imaginable but I just love the things like the Willow Tree and the ornament that we can still enjoy.

As a twin mom, no joke, the nicest gift was someone to watch the babies for a few hours in the early days so that I could nap. So. Tired. As another twin mom, I heartily agree. Other standout gifts I still rave about (and my twins are in HS) was the acquaintance who swooped in and thoroughly cleaned my kitchen and baths a month after the babies were born, my mom's cousin who helped me at the pediatrician office when the babies had to go every week or two, and the neighbor who mowed the lawn knowing we were too overwhelmed to do it. Honestly, I didn't need more stuff. And now I'm trying to figure out what to do with a bunch of "special" little extras that I was told the kids would treasure (they're not interested; items weren't handmade or old heirlooms). Give a gift of your time. It will be a much needed help, and it will long be remembered as the perfect gift.

Still do all these years later, but I had no time and was lucky I remembered to take photos things were so hectic! I agree. The diaper service we got for the first 6 months was great. After that we switched over to the diapers we had saved from our firstborn.

Get them a gift card to a photo studio. I loved that gift. I will have the picture forever and I enjoyed being able to get the pics taken at an early age!
