Just shipped for the first time in quite a while & noticed Smiles no longer takes Birthday cards. Have 72 signed & ready to donate - they’re more for adults than kids so Confetti Fdn doesn’t seem quite right. Any recommendations?

I think that if you read the fine print, they will take them, they just don't prefer them. I know they prefer signed cards to unsigned cards as well, so I'd probably reach and see if they'll take them.
Cards for Kindness takes adult birthday cards, but requests unsigned specifically.
Maybe reach out to nursing homes, senior centers, and similar facilities in your community to see if someone can use them?
Cards for Cubs serves young adults in foster care as well as kids, but you'd have to hang to them for a full year.
EtA: If you adult birthday cards are fully signed BStS may also be able to find a senior community you can send them to directly.

They may have a suggestion of who can use them. They’ve always been really helpful when I’ve had a question. But otherwise saving them for Cards for Cubs if they could be teen/young adult suitable might be your best bet.

I think you’re sending to the Oregon/western address now the same as me. Yes, the beauty of BSTS is they accept all holidays year round and store-bought cards. They certainly don't mind well re-made used cards, which is why they get so many of my recycled Pea exchange cards.
I was having a real dilemma once I joined the Pea birthday list, though, since they don't accept birthday cards anymore.
BUT, Cards for Kindness has assured me that handmade birthday cards, remade on fresh card bases, with no inside sentiments or writing, ARE acceptable!
Also that "sending smiles" "have a great day" "just for you" and greetings of that nature all count as "thinking of you" for the purposes of their transmission form.

I'm spending some of today remaking Pea card exchange cards for Bring Smiles to Seniors, since they take all random holiday cards, which Cards for Kindness really doesn't. I mean, they may have a brief window when they accept Halloween cards or whatever, but who has time to chase that down all the time? BSTS doesn't like completely blank cards, though, so I'm making sure to stamp a sentiment on the insides. I'm looking at my fall/autumn non-Halloween cards now and wondering whether to stamp a generic fall message or Happy Thanksgiving. I'm leaning a little toward the latter. Thoughts?

I ended up going with the Happy Thanksgiving "innie". I decided that while BSTS takes pretty much all hello/thinking of you types of cards (though they prefer them with inner sentiments) and pretty much all holidays, autumn greetings or similar fell into a void where it was seasonal and needed to be sorted in a way that a generic hello/thinking of you didn't. For that reason, it seemed more fair to the organizers to call them Thanksgiving cards.
But now it has me second-guessing how I'll remake spring and summer cards. Though I suppose many spring ones can be rehabilitated with an Happy Easter innie....

Club Scrap promises fully decorated cards ready to send with no stamping required. Which is great, if it’s a card you like and will use. If you later decide you want to donate it, it can be an issue, since most organizations want blank or mostly blank insides. I assembled a set of four this morning that I think could go to Cards for Cubs as encouragement cards: I’m thinking that “dreams are like fireflies” really needs its innie (“chase a few and they’ll light up the darkness”), but I think the other three scan fine. Thoughts? I can list the other innies if it helps (and I’d probably trim them closer and definitely skip the other inner decor).

I agree that the dreams one could use an innie otherwise it might not read as inspirational.
